SPECIAL CONDITIONS

The plan is compulsory to all Principal Members

Cessation age of 95 years

Maximum entry age of 94 years.

One benefit structure per group

Cover ceases on withdrawal from the plan or death of the Principal Member, or cancellation of the plan

Principal Members must be actively at work at commencement of cover.

Fully completed individual application forms must be submitted at inception, and immediately when new persons to be insured join.

Membership data must be submitted monthly in support of payment made.

No waiting period for claims in respect of the Principal Member, Spouse and Eligible children

10

A 6 (six) month waiting period will apply for claims in respect of Extended Family Members.

An Annual Review of the plan will occur in January every year.

12

Only 2 stillbirth claims will be accepted per family during the term of the policy Death due to suicide will not be covered during the first 24 months after the cover commenced for the individual.
